Reporting Workplace Injuries

In an emergency always call 911

If your business is covered by the Occupational Health and Safety Act. (OSHA) and there has been a workplace incident or illness including death, loss of consciousness, critical injury or occupational illness you must notify the following even if the person is not a worker:

  • The Ministry of Labour, Training and Skills Development’s Health and Safety Contact Centre at 1-877-202-0008 (operates 24/7)
  • The Joint Health and Safety Committee or health and safety representative
  • WSIB within 3 days of learning of any illness or injury, file Form 7

 

WSIB Reporting

You must report workplace injuries or illnesses to the WSIB, if the injured worker:

  • requires treatment from a health professional (beyond first aid), or
  • is absent from work (e.g., if the worker has to leave work for any reason pertaining to the injury or illness), or
  • earns less than regular pay (e.g., being paid less per hour or working fewer hours).
  • does not receive healthcare and requires modified work due to the injury or illness and has been doing modified work at regular pay for more than 7 days. In this case, the reporting obligation begins on the 8th day of modified work.

    Note: Modified work is assigning different duties or changing a job to accommodate a workers injury while they recover.
